Is there a fee to use the Patient Portal?

The Patient Portal is a free service offered to our patients.

How do i get started with the Patient Portal?

You will need access to a computer and the internet. To get started, please go to our website, www.horizon-med.net, and click the link for patient portal. We will provide you with your own personal activation code included in a letter provided by the office.

Is my access code my User ID?

No, you will use this code only once to log in to the patient portal for the first time. (The code expires after you have used it or after 30 days). When you log in to the portal for the first time, you will need to create your own unique Patient Portal ID and password.

What information will I be able to see and can I update my information on the Patient Portal?

The patient portal is for NON-URGENT issues.  You will have access to your medications, request refills, see lab results, message your provider, request appointments, and update your personal information.

Why can't I see my test results through the Patient Portal?

Test results are released to your patient portal account after your physician has reviewed them. This is generally in 10-14 days. Your provider determines which types of test results can be accessed through the Patient Portal. Test results of a very sensitive nature are not released to the Portal.

If I send a message to my doctor or nurse, when can I expect a reply?

You can expect to receive an answer within 1 to 3 days. PLEASE NOTE THAT THE PATIENT PORTAL SHOULD NOT BE USED FOR URGENT ISSUES. Please dial 911 if it is an emergency.

Is the Patient Portal secure?

Yes, your health information is kept private and secure.  Each person controls their own password. The Patient Portal uses encryption technology unlike conventional e-mail.
