AT YOUR APPOINTMENT

At Horizon Medical Corporation PC, to help maximize your visit, we ask you to do the following:

15-minArrive early

Arrive at least 15 minutes prior to your appointment time.  If you arrive more than 20 minutes after your appointment time, we cannot guarantee that you will be seen. Late arrivals may need to reschedule their appointment.

April 1 calendarHealth Insurance Card

Always bring your current insurance ID card to each appointment. A front desk staff member will ask if any of your personal information or insurance has changed.

HMC participates with many insurance plans.  Please call your insurance company to verify that HMC participates with your plan or to obtain prior authorization if our physician that you will be seeing is considered an “out of network provider”.

Rx Prescription FormList of Current Medications

It is important to bring a list of your current prescriptions and any over the counter medications.

CancelledCanceling or Rescheduling an Appointment

If you are unable to make your appointment or need to reschedule for a later date, please notify the office at least 24 hours prior to your appointment.

Health and medical recordTransfer Previous Medical Records

Bring previous medical records with you to your appointment or request the records to be sent to the location of the HMC provider that you will be seeing.
